How the assessment works

You answer 12 statements about the change, choosing from Not in place to In place and verified. Choose based on evidence, not intention. Not sure is a valid answer: it flags a visibility gap, not proof of failure.

Each answer scores 0 to 4, and every question counts equally. Not sure scores 0 because the evidence could not be confirmed, and the results show how many answers were Not sure. The bands are design choices to help you prioritise, not industry benchmarks or predictions.

Your answers stay in this browser tab and are normally cleared when you close it. They are only sent to our server to create a PDF or an email you ask for, and we do not store them. An emailed report includes your answers and is delivered by our email provider. Read the privacy notice.
